CEU Certificate Packet Forms

CEC Sponsorship

Organizations outside of the University requesting the Office of Continuing Education and Conferences to sponsor a Certificate Program must complete the form below. This should be completed before submitting the request forms for courses and certificates.

Submit Courses for Approval

Each course associated with the certificate must be submitted for renewal or approval. After each submission, you will be given an activity code to input into the certificate request form.

After receiving the assigned activity codes for the related courses, submit the form below to provide information regarding the proposed certificate.
 

Certificate Request

After all forms have been received and reviewed, you will receive a confirmation email with the updated status and due fees.
